SUMMARY:
The Nurse Practitioner (“NP“) will have a certificate of completion from an approved program for nurse practitioners, be certified by the ANCC or AANP, and maintain such certification in good standing at all times, will be licensed as a registered nurse and authorized to practice as a nurse practitioner in the State of and maintain such license and authorization in good standing at all times. The NP will report to the Supervising Physicians and/or the Medical Director. The NP will be responsible for meeting CEU requirements to maintain certification and, licensure, and authorization in order to assist Human Coalition achieve its mission of existing to rescue children and serve families across America. Human Coalition is an innovative, energetic, compassion-driven non-profit organization with the purpose to end abortion.

ESSENTIAL FUNCTIONS:
• Perform history and physical exams on new and established patients and those being seen for follow up appointments. Consult with the Supervising Physician(s)/Medical Director as needed.
• Initiate appropriate treatment and testing required for evaluation of illness based on training and experience.
• Provide counseling and instruction to patients regarding health-related issues.
• Document clinical care of the patient according to established guidelines.
• Provide follow up and health maintenance care to patients in accordance with established protocols or specific instructions from the Supervising Physician(s)/Medical Director.
• Perform therapeutic and diagnostic procedures as indicated by training and experience and as directed by the Supervising Physician(s)/Medical Director.
• Communicate with patients and family members within state law confidentiality and HIPAA guidelines concerning health related questions.
• Participate in continuous Quality Improvement process and other initiatives in the area of Employee’s practice.

PERFORMANCE STANDARDS:
I. Executes health assessment and gross physical exam.

Expected performance standard:

a. Appropriate health/medical history is obtained.
b. Patient’s perception of problem and information about patient’s physical environment is obtained.
c. Body systems are reviewed, initial physical exam is performed, and screening examinations are completed.
d. Psychosocial and family history are observed and assessed.
e. Appropriate testing and exams are ordered.
f. Deviations from the normal are identified.

II. Identifies potential health problems, risk factors and/or complications.

Expected performance standard:

a. Health habits are assessed, and risks are identified.
b. Patients are included in a discussion of findings.
c. Patients are educated about management of the findings.
d. Physician consultants and referrals are appropriate.
e. Continuing or recurrent problems are addressed.
f. Emergency medical problems and managed and/or emergency procedures initiated.

III. Collaborates with physicians and other health professionals.

Expected performance standard:

a. The health plan and diagnostic work-up is implemented.
b. Relevant patient education supporting (a) is done.
c. Patients are encouraged and supported to be responsible for their own health behavior.
d. Patient triage is appropriate.
e. Nursing is advocated and supported at all levels.
f. Other health professionals and resources are identified for the patient as indicated.
g. Follow-up and evaluation of all services provided results in coordinative health management and education.

IV. Organizes and communicates relevant data about patient.

Expected performance standard:

a. Data about patients is documented systematically and accurately.
b. Patient charts convey accurate information.
c. Diagnostic and laboratory tests are interpreted accurately, and appropriate information is shared with patient.
d. The NP acts as a patient advocate.

V. Provides assessment, planning and intervention for acute minor illness and minor trauma per protocol.

Expected performance standard:

a. Unusual incidents/changes in patient’s condition are reported to physician.
b. Continuity of health management is provided.
c. Therapeutic regiment is supported.
d. Constructive health behaviors are promoted.
e. Patients are educated about and encouraged to manage and live with their chronic disease or disability.
f Long range plans are developed with patient for restorative/rehabilitative activities.

VI. Attends selected meetings, belongs to appropriate committees and supports Employer policies and procedures.

Expected performance standard:

a. Recommendations for improving care are made.
b. Change is suggested and objective measurable data to support rationale is provided.
c. Periodic meetings with physician preceptor(s) are arranged.
d. Professional association membership is maintained.

EDUCATION AND EXPERIENCE:

• Active Specific State license
• Nurse Practitioner Board Certification
• Master’s degree in NP studies from approved nurse practitioner program
• Master’s Degree Nurse Practitioner Req
• 1 Year as a Nurse or Nurse Practitioner req

• Tasks involving exposure to blood, body fluids, or tissues.
